PJS Leads Alegent Creighton Health to Success in Securing CMS Grants

In the past six months, Paul J. Strawhecker, Inc. has assisted Alegent Creighton Health in securing two competitive grants presented by the Affordable Care Act. Both multi-million dollar grants were awarded through the Centers for Medicare & Medicaid Services (CMS).

The first grant, announced this past spring, was awarded to UniNet in partnership with Alegent Creighton Health and the Eastern Nebraska Office on Aging for the creation of a Community-Based Care Transitions program to reduce the rate of preventable hospital readmissions through a program of post-discharge care management. The funding for the program, known as the UniNet Discharge Transitions Program, was awarded for two years with an option for renewal for an additional three-year award based on the achievement of established goals.

The second grant, announced in late September, was awarded to Alegent Creighton Health in partnership with the State of Nebraska and 15 local nursing facilities. The grant, the Initiative to Reduce Avoidable Hospitalizations among Nursing Facility Residents, is focused on interventions to reduce hospitalizations and improve the health outcomes of long-term care residents of nursing facilities. The program developed will provide nurse practitioners on site at the partnering nursing facilities, focusing on improved medication management, improved oral health, improved communication among the healthcare team and 24/7 clinical support for physicians and nursing facility staff. Alegent Creighton Health is one of only seven awardees of this grant across the country. Funding will be provided over a four-year award period.
